Milestones by area

General rules

  • Each domain has blocks. Within a block, work on one milestone at a time. Move to the next milestone only when the current one is achieved.
  • Several blocks can be worked on at the same time. The one-at-a-time rule applies within each block.
  • Every block starts from its first milestone.
  • Any family member or attendant can tick a milestone when it is achieved.
  • If a ticked milestone was ticked by mistake, untick the latest tick in that block, or tap Undo straight after ticking.

Two sessions every day, each about 30 to 40 minutes, in this order:

1Sensory Integration (SI)5 to 6 min
2Cognitive therapy10 min
3Gross motor activity5 min
4Language + cognitive activity5 to 10 min
5Sensory Integration (SI)5 min
  • Do not do all activities from the same category in a row. Alternate across different milestone groups.
  • Do not over-practise a single activity for an hour. After 10 minutes of cognitive work, take a break and change the activity.
  • Milestones are interconnected. Achieving them in sequence makes it easier to reach the next one.
  • Music and dance: 5 to 10 minutes of free play daily. Moving to rhythm and copying actions supports connection and imitation.

1. Imitation

  • Starts with motor imitation (copying actions), then moves to verbal imitation (copying sounds and words).
  • Use a mirror.
  • Copy Pranshi's actions to build connection.

2. Picture identification

  • Target by three years: at least five items from each category, including animals, birds, fruits, vegetables and vehicles.
  • Flashcards: place three cards in front of her and ask her to hand over the named card. Guide her hand at first.
  • Match real objects to flashcards before introducing naming.

3. Body part identification

  • Target by three years: at least six to seven major body parts.

Colour, shape and size

  1. Matching
  2. "Where" questions, with pointing
  3. "What" questions, with naming

Object persistence

  • Hide a toy under a cloth.
  • Hide a food item she likes under one of three bowls, shuffle them, and let her find it.

Social games

  • Play one-two-three-go while holding her hands. Do not let go until the cue word is spoken.

Gross motor

  • Keep practising milestones she has already achieved, such as side walking and backward walking, as imitation tasks. This combines cognitive and motor goals.

Ring sets and beaker sets

  • Use both ring sets and beaker sets. Square shapes are harder than circles because a side has to be aligned, so get both.

Sensory integration

  • Compulsory: tactile, vestibular and proprioceptive.
  • Auditory and visual components are not needed for Pranshi.
  • Once a week, for example on Sunday, go through all achieved milestones (the ticked ones) to reinforce them and prevent regression.
  • If any milestone is not clear, note it and send it to Dr Patel. She will reply with short audio or video clips.

Focus on the next three months of this plan. Worrying about school placement at age five is not the focus right now.
